Wasn't sure if I should post this here or in the calibre forum.
Occasionally, when I sideload an ePub, using the calibre Kobo plug-in to convert to kepub as it sends, the kepub will have double the number of pages and the book will start in the middle. So I bypass the cover, copyright, etc. and chapter 1 is on page 318 out of 637.
Loading the same ePub on my 13 year old Nook STwG, everything works correctly.
