MR Plugin Index updated

Because they were not being maintained the fields that are also in the plugin code have been removed from the index - version, release date, minimum calibre version etc.

For most plugins the index entry consists of the PI thread link and description, an optional Donate link can be used; and the Uninstall and Deprecated fields are retained.
